Privacy Information

At this practice, we understand that information you provide to us may be of a personal and sensitive nature. We will therefore take all reasonable steps to protect the privacy of the personal information contained in our medical records in accordance with the obligations under the Victorian Health Records Act 1.7.02 and Commonwealth Privacy Act 19.12.01.


Information that is held within your medical file is collected in order to evaluate and assess the cause of your condition, provide a diagnosis and appropriate treatment plan. Every staff member will have access to this information.


This information may be provided to other healthcare professionals, such as treating doctors, pathology services, radiology services and other specialists as deemed necessary by Greg Malham.


Information may be required for administrative purposes, such as medical indemnity insurance, provider information, billing, hospital bookings, health insurance funds, Health Insurance Commission (Medicare) and other financial organizations responsible for your care such as the Transport Accident Commission/Workcover.


Information may also be used for conducting research programmes and the ongoing training and education of other health professionals.

We will endeavour to ensure that your information is accurate and complete. Please contact us immediately if information you have provided to us changes.


We will take all reasonable precautions to protect the security of the personal information that we hold, whether in hard copy or on computer records.

Access to your personal information will be provided in accordance with the Victorian Health Records Act 1.7.02 and Commonwealth Privacy Act 19.12.01. Should you require access to your medical records, please contact the Practice Manager.



If you have any complaints about the privacy controls of this practice and how your personal information is managed please feel free to discuss this. All complaints should be in writing and will be handled in a professional manner. If you are dissatisfied with the outcome from your complaint, you may contact the Victorian Health Services Commissioner or the Federal Privacy Commissioner.
